Sometimes genetic metabolic abnormalities produce body changes that are uninvited. This 23-year-old female had a congenital enzyme abnormality that interfered with her production of estrogen. Instead she made excessive testosterone. She was interested in mastectomy so that she could be identified as a male. She had 36D breasts. She underwent mastectomy with free nipple grafts. Her postoperative course was uneventful. The photographs are six weeks after surgery. The incisions are still red, but these will continue to improve over the next several months. The shape and symmetry is excellent. He is very pleased with the results.
