This pleasant 51-year-old female had lost almost 90 lb by changing her diet and increasing her exercise. In addition, she had been involved in a clinical study using an appetite suppressant. The massive weight loss had left her with loose skin on her extremities and trunk. The plan was to address her extremities and trunk in three staged procedures. The first stage procedure would address the thighs and trunk. The second stage procedure would address the upper trunk including the back and the arms. The final stage would address the breast reconstruction. Preoperatively, she wore a size 20 pant size. Her waist was 39¼ inches and the circumference at the umbilicus was 48¾ inches. Her hips had a measurement of 49½ inches and her measurement in the mid thigh was 26½ inches. The markings for the procedure were placed in the office and can be the seen in the photographs. Liposuction of the trunk and thighs were performed initially, removing almost 4000 mL of fat from these areas. A lower body lift was performed along with an inner thigh lift. The inner thigh lift incision was placed on the inside of the thigh from the knee to the groin area. The total weight of the tissue removed from the thighs and the trunk was 13 lb. She spent two nights in the hospital. She developed an infection in the abdomen and this required two procedures to address the infection. Despite the infection she went on to heal without additional problems and obtained a nice result. The second stage procedure was performed about six months later. Since her initial trunk and thigh surgery, she had lost an additional 40 lb. She underwent an arm lift and removal of loose skin on the upper back. Liposuction was performed with the arm lift and the back lift. The weight of the tissue removed from the arms was approximately 1 lb and the weight of the tissue removed from the back was about 1½ lb. A small revision was performed on the tummy tuck. She had no problems with the second procedure. The photographs taken eight weeks after the second surgery show nice improvement in the contour of the arms and the loose sagging skin on the upper back is gone. She has a beautiful shape to her abdomen. Her weight is now 125 lb. She wears size 4 pants. She recently got engaged and plans on getting married in the near future.
